International Health Certificate Questionnaire

International health certificates for the export of animals from Nigeria are completed by an accredited veterinarian, who certifies herd and animal health status, conducts tests, and records test results for the individual animals being exported. Complete and signed international health certificates for the export of animals from Nigeria must be endorsed by a Veterinary Services area office in order to be valid.

Important Information:

Please include Rabies certificate*, any vaccination history, and proof of microchip implantation (date) if the treatments specified were not performed here at BlueBlood Vet.

*Original copy signed in blue ink by the Veterinarian that administered the vaccine with the microchip information on the certificate

• If the pet requires a booster of a Rabies vaccination, it must be given 21 or more days ahead of departure date.
• Pets without microchips will need one prior to a Rabies vaccination in order for the Rabies to be valid.
